VOLUNTEER LIVE SCAN FINGERPRINT INFORMATION

1. Please go to this website ( https://www.applicantservices.com/ayso ) to make an appointment to complete your
Live scan Fingerprint check after you have completed the Volunteer application on our website at:
( aysoregion657.org )

2. Click on the ( Get fingerprinted at a Live scan Service Center ) do the paperwork,
Schedule your appointment to get it done, pay the $25 fee,
copy your receipt and Region 657 will reimburse you for your cost.
(you have to submit a receipt and paperwork with your name on it to receive a reimbursement)

3. You can submit these receipts per email if you like at ( [email protected]) or
you can hand carry your paperwork and give it to Zaimah at Region 657 at the soccer field once
practices have started.
